Our aim is to help navigate families during what can be an overwhelming journey. We create a support system that allows families to focus on their loved ones while we coordinate and manage their care. Our Palliative Care Program is a combination of Palliative and Home Health services jointly managed by our diverse team of healthcare professionals’ doctors, nurse practitioner, nurses, home health clinicians, physical and occupational therapists. Palliative Care provides relief from pain and other distressing symptoms while integrating the psychosocial and spiritual aspects of patient care. Palliative Care can be helpful at any stage of illness in conjunction with curative treatment such as chemotherapy and radiation. Palliative Care can also help patients and families understand what choices they have for medical treatment.
