My career has done a full circle of staying within the healthcare field. After many years of searching what is the perfect fit, I’ve found my home. Working with True Care Home health and pouring my heart into patient care has been a wonderful journey. As the intake coordinator for True Care Home Health, I am the first person that everyone speaks to, to assist in coordinating the best plan of care. I’ve had such high compassion for others that I find myself appreciating the stories of my patient’s ongoing journey from old to new diagnosis, managing mobility, and even just the hardship of living a full life with an illness. I aim to get each patient back to their normal lives..
One of the key factors we do to ensure the patient’s and family’s expectations are met is creating a Plan of Care for each individual case. Developing the care plan with families is a detailed step we take, which includes but not limited to speaking with the doctor who is requesting the care to get information on the patient’s current health and receiving recommendations for the patient care plan. We then proceed to contact the family, to get to know the family to see what nurse fits best to meet their needs and expectations. Please note, your doctor will be updated on progress throughout the duration of the program, which is determined on a case-by-case basis. I have found that the first conversation with our families is the most important conversation we have, and the first RN visit shows the family how we truly care!
I have noticed over time that families or patients are hesitant to enter our home health program, whether it be they may not understand the program, or accepting the help, or simply having someone visit their home without knowing what visits can entail. A lot of clients tend to question the services being provided and health concerns when it comes to caring for their loved ones. This is where we find if the patient or the family would like us to investigate more resources that are available to them through the program. Our program offers the following and not limited to:
- Skilled nursing: RN or LVN
- Physical Therapy
- Occupational Therapy
- Social Service
- Home Health Aide
- Laboratory Tests
However, by the end of the program, most of my patients are ready to get back to their routine and are very comfortable each step of the way. Personally, it has been most rewarding to have been able to speak with clients and educating them on how to improve their quality of life. True Care Home Health and Palliative has been the heart of it all. As a member of the team, I attempt to make every patient get the VIP treatment they deserve. Every patient is different and each one deserves the best care. I am proud and honored to be able to provide that to each of our patients through the mountainous number of resources we have to offer.
